Get Started with Your Provider
Now that you have your member ID card, it’s time to find a new provider, or update your insurance information with an existing provider.
If you need any assistance in doing so, before your next intended visit, call the dedicated number on your ID card and one of our Concierge reps will be happy to assist.
Bring Your ID Card to Your Visit
When it’s time to see the doctor, be sure to take your ID card. You will be responsible for any copay or co-insurance amounts during your visit, as applicable according to your Employer plan design.
Claim Processing
Following an appointment, your healthcare provider submits a medical claim to Vitori Health as your Plan Administrator. We will review the claim to ensure it is accurate and determine the amount to be paid by the Health plan based on your benefits.
Review Your Explanation of Benefits (EOB)
After your claim is processed, you will receive an explanation of benefits (EOB). This is not a bill, but a summary statement that lists details of the services your healthcare team provided and how your specific medical benefits were applied.
Be sure to pay close attention to the “Patient Responsibility” section, and do not pay any amounts that are higher that that applied responsibility on the EOB.
